Welcome to the Ontario Health Indicator Library

The Ontario Health Indicator Library provides a fully searchable library of indicator profiles reported by OH and other entities. The library will feature indicator characteristics, including: the indicator definition, the rationale as to why (or why not) an indicator is reported, technical specifications, alignment information, information about data sources, and links to those sources. In addition to a keyword search, you can perform side-by-side comparisons of up to three indicators, based on different filters, such as sector, dimension, type, topic, and data source. You can also export the indicator definitions to a format of your choice.

Emergency Department Visits by Home Care Patients within 30 Days of Leaving Hospital	This indicator shows the percentage of new patients receiving publicly funded home care who had an unplanned emergency department visit within 30 days of leaving hospital, among those referred to home care from hospital or within 7 days of discharge. Timely follow-up after hospital discharge can help prevent the return of home care patients to the emergency department.
