Description
This indicator measures the Percentage of patients who have had a 7-day post hospital discharge follow-up, by a primary care provider (physician or nurse practitioner) for the following conditions: pneumonia, diabetes, stroke, gastrointestinal disease, congestive heart failure, chronic obstructive pulmonary disease, and cardiac conditions.

Calculation Methods
The percentage is calculated as: numerator divided by the denominator times 100.
Numerator (short description i.e. not inclusions/exclusions)
Number of discharges where the patient was seen by a primary care physician within 7 days of discharge from hospital for the following conditions: pneumonia, diabetes, stroke, gastrointestinal disease, congestive heart failure, chronic obstructive pulmonary disease, and cardiac conditions excluding heart attack.
Denominator (short description i.e. not inclusions/exclusions)
Number of acute care discharges for an episode of care in which one of the mentioned conditions is recorded in the first hospitalization of the episode within each fiscal year (minus 30 days for follow-up).
