Welcome to the Ontario Health Indicator Library

The Ontario Health Indicator Library provides a fully searchable library of indicator profiles reported by OH and other entities. The library will feature indicator characteristics, including: the indicator definition, the rationale as to why (or why not) an indicator is reported, technical specifications, alignment information, information about data sources, and links to those sources. In addition to a keyword search, you can perform side-by-side comparisons of up to three indicators, based on different filters, such as sector, dimension, type, topic, and data source. You can also export the indicator definitions to a format of your choice.

Premature is defined in Canada as deaths before 75 years of age. Complications Among People Living with Diabetes	This indicator measures the rate of serious chronic complications (death; hospitalization for coronary artery disease, cerebrovascular disease, or peripheral vascular disease; dialysis for end-stage kidney disease) and death in the last year among people with diabetes aged 20 and older. Same-day response to phone call to a Primary Care office	This indicator shows the percentage of people aged 16 or older who reported that when they called their primary care provider’s office with a medical concern during regular office hours during the previous 12 months, they always or often spoke to their provider or someone in their office on the same day, in Ontario. Getting a timely response from a primary care office can lead to better-organized care and greater patient satisfaction. Patient involvement in decisions about their care	This indicator shows the percentage of people aged 16 or older who reported that their family doctor, or someone else in their office, often or always involved them as much as they wanted in decisions about their care and treatment, in Ontario. People who are involved in decisions about their own care generally have a better experience with the health system. Having a family doctor or other primary care provider	This indicator measures the percentage of people in Ontario aged 16 and older who reported having a family doctor or other primary care provider for regular check-ups and when sick. Having a primary care provider is associated with better follow-up, screening and illness prevention for patients, as well as better use of resources within the health system.
