Response options: 1. Extremely satisfied, 2. Very satisfied, 3. Moderately satisfied, 4. Slightly satisfied, 5. Not at all satisfied, 6. Multiple-response code, 7. Decline to answer

Denominator including inclusion/exclusion

Weighted number of respondents to the survey question above who answered "slightly" or "not at all satisfied."

Exclusions:

1. Those who answered “multiple-response code” and “decline to answer.”

Adjustment (risk, age/sex standardization)- detailed
The primary care physician (PCP) data in Canada were weighted to account for: (1) the over-representation of PCPs in some provinces; (2) the availability of an email address or not (since respondents with email addresses could be contacted both by mail and email); and (3) differential nonresponse along known geographic and demographic parameters.
