This is an additional QIP indicator for 2018/19. Current performance reporting period: Annually Primary Care Group Practice reports are available to family health teams (FHT) that register. For more information visit www.hqontario.ca/pcreport. Primary care organizations not registered to receive group practice reports will be required to extract data from EMRs. Equivalent measures are available for CHCs, nurse practitioner-led clinics and AHACs, extracted from electronic medical records (EMRs). This indicator was retired in the 2019/20 QIP.
