Welcome to the Ontario Health Indicator Library

The Ontario Health Indicator Library provides a fully searchable library of indicator profiles reported by OH and other entities. The library will feature indicator characteristics, including: the indicator definition, the rationale as to why (or why not) an indicator is reported, technical specifications, alignment information, information about data sources, and links to those sources. In addition to a keyword search, you can perform side-by-side comparisons of up to three indicators, based on different filters, such as sector, dimension, type, topic, and data source. You can also export the indicator definitions to a format of your choice.

Emergency Department Visits by Home Care Patients in the Last 30 Days of Life	This indicator shows the percentage of patients receiving publicly funded home care who had an unplanned visit to the emergency department in the last 30 days of life. Such visits can be an extremely difficult experience for patients and could indicate they did not receive the care they needed in the community.
